Spain: 20410 cases; 1041 deaths
Germany: 18450 cases; 53 deaths

despite the much better healthcare system in Germany, there has to be some kind of difference on what a "death from coronavirus" means in each country

Germany has more ICU beds per head, more ventilators and more PPE for front-line health care workers. Italy got swamped by day 7 and had to start turning away chronically ill people as they couldnt offer them any ICU help or support. Then to cap it all off they started losing their front-line docs and nurses to the virus.

I think Spain is in a similar position to Italy - and we are an in an even worse state

Thats what its all about...ICU beds, ventilators and staff.

Apparently if any dies at home they do not do retrospective coronavirus tests, which may affect the numbers reported.....
