I'd say it would be more down to dropping points to relegation candidates and already relegated teams several times that did us in, rather than ending up with a point against potential Champions, but you do you.
Of course there's moments all over the season from the manager and individuals which have cost us, but we are moments away from the arguably the biggest result of the season and he demonstrates every bit of that lack of intelligence he's been accused of by heading the ball back into City's possession, when he could have just run the ball into the corner and then, from the resulting corner he defends like a fairy. All on him.

I'm reacting in the heat of the moment about him being most directly to blame for not qualifying for Europe, of course, but it's a pathetic 30 seconds which directly costs us 2 points, with 3 games to go. It's hard to look past it as hugely significant
 
Yup, he was first to the ball after a clearance, was there to clear the ball on other occasions. He was living upto Moyes's credo of head the ball in both boxes, what more do you need.
Moyes's credo isn't to commit two mistakes in 30 seconds at the end of the game which cost us 2 points with 3 games of the season to go.
 
Heads a ball back into the middle of the pitch when he comes have headed it wide, ran onto it and ran the clock down to win the game.

Instead allows the final attack and then is solely to blame for the way Doku is allowed to get his shot away

It was absolutely bizarre why he done that header. Like you said, if he heads it wide he runs onto it and goes to the corner flag.

Ndiaye missed a 1v1 badly and afterwards selfishly didn't pass to Rohl who was staring at an open goal, 3m away, so he's directly cost us 2 goals.

Was what Alcaraz did bad? Yeah, a bit brainless, not unexpected from someone who's not seen meaningful first team minutes in months, but brainless nonetheless.

Is he at fault for everything bad this season? Cmon mate.

I didn't see any glaring mistakes to be honest but yes, we will or should be seeing more of him. Needs match fitness still looks like, and that unsuccessful run through on their defense must have changed his mind on making that pass instead. Must have got the instructions (if any) to push for a fourth, which is positive for Moyes.

No he isn't, but it's a horrible 30 seconds from him at a crucial moment in the season. It's killed us. One mistake is hard to take. The mistake to gift City the attack and then the defending for the Doku shot is a kick in the balls. It's hard to take
