If I go for a middle aged health check up, let’s say there is 20 questions, with questions exclusive 10 to males, 10 exclusive to females, based on the difference between males and females.

I was leaving the next point, as my grammar is not always the best for gods sake, when you are talking about the plural for female humans it is WOMEN, not WOMAN. In the context of sports and spaces the more correct term would probably be female as it flows better that women. You are right sex is generally an irrefutable fact, although there are rare cases where it cannot be scientifically determined. However, gender is subjective and this is what you have to understand. People think differently to you, people have different feelings to you, why can't you accept that.
In rare cases, there will be times when a persons personal belief/emotional feelings, towards their own gender identity, will affect another persons, but 99.9999999% of the time it affects nothing and nobody.
