There's a distinct difference in his speech pattern from his presidentiel run and presidency compared to older clips and appearances.
Been saying it for months.
You look at footage of him from the '80's or even just ten years or so ago, and he was reasonably quick and articulate.
The difference is quite shocking. He's clearly not got the full use of his mental faculties.
I don’t think it has to be an either/or.
He could well have narcissistic disorder (the description of “an inflated sense of their own importance, a deep need for excessive attention and admiration, troubled relationships, and a lack of empathy for others” sure fits his history) that is being further complicated by early onset dementia.
As @Fedlingen says there is substantial deterioration from his youth and given 1 in 7 Americans have dementia at 70 it wouldn’t be a surprise.
Of course neither are particularly reassuring in a guy who controls nuclear weapons.

Honest to god, I hope presidential candidates are forced through a full medical.
No clue, how it works though.
Usually the candidates doctor writes a letter describing their health, Trump’s was unconventional to say the least...

If anything that article shows that his doctor needs to be looked into professionally. All of the articles with factual information about this Bornstein that i have read are astonishing to say the least. This doctor seems not to keep his house in order, lives of his Dads reputation and make audacious claims about his own life. That and the whole letter thing was simply odd just like this man.
He comes across as being as nuts as Trump.

The thing is if its all lies why not simply show your tax return or take a proper medical or prove the crap one says day to day. When they pestered Obama on his birth cert he produced it. He didn't have to humor them but he did anyway.
The reason the whole rhetoric of the "fake news" came along is because he needed to convince his supporters that any factual information the media produced was fake. That way he could say, do anything he wants and turn it back on the media on any indiscretion from his past.
If there is one thing and i mean one thing you can credit him for as in his use of his actual "smarts" it would be the fake news agenda.
If I recall correctly, the most thorough medical exam was from John McCain---probably a necessity given his age, family history, and relative youth of the opposition
