Martinez has told them to play out the back, to keep the ball, not to hoof it out. So on both occasions, why does the keeper or any other player show for him?
Howard is stuck in his 6 yard box. Sees Stones hasn't got a pass, hasn't hoofed it, and he's still stuck in that 6 yard box. He did the same against Spurs.
When the style of the team is there for them to carry out, the players should all do. Stones is just doing what he's told to do. All Howard has to do is show for the ball, step a yard or 2 for the pass, and kick it.
