  • They don't have enough tests for all self isolating NHS workers
  • Each trust does have a list of such people
  • Critical care staff are being prioritised for tests
Just seen your last point. I'd prioritise those at home if it were me. get those that you can back into work. Plus you know they either have symptoms or live with some that do.

If you test critical care workers, the test doesn't come back for 4 days anyway. Do you then test them again?. Or do you test them daily on a rolling basis, or even tice daily. They can catch this thing at any time in theory. Until they get their hands on immediate testing capability I wouldn't be testing anybody until they started showing symptoms.

Even so, I'd still be asking why these critical care workers haven't been able to get to a testing centre if they're the ones who should be going there.

Interestingly mate, despite initial evidence that masks were useless, there is a growing body of evidence that they can be an important part of defense and attack against the virus.

While they only really offer close to 100% protection of infection transmission to those who have the virus sharing with someone else, if everyone wore masks, infected or not infected then it offers a significant defense against droplet community transmission.

Particularly as we know there is a reasonably high percentage of patients who are asymptomatic but can spread the illness and people who might have mild symptom that can still spread the virus. In other words if everyone wore a mask to protect everyone else its likely to have an impact on transmission, a you prtect me and ill protect you approach of everyone wearing masks. Equally in terms of exposure to droplets, the virus has to work harder to penetrate two masks and 1 meter as opposed to 1 meter with out a mask. Essentially if wearing masks was the norm in society it could have impact an on community transmission. Its a growing thinking.

I mention this as mask wearing isnt universal in the west up to this point, its far more more common in the east, in places like China, Singapore, Japan and South Korea were they have done really well to flatten the curve, while keeping the economies and communities mobile. Mask wearing may be an important part of managing the initial and secondary surge.

If Mask wearing became the norm in communities for the next year or so, it could go a significant way to managing the virus especially community transmission, but would be a collective effort and not just the odd person you see.
The “don’t wear masks“ recommendation seems largely to be because (at least in US) we are too short of them for healthcare workers.


Hopefully we get our act together on producing more so both medical staff and general public can wear them as agree that could be important part of managing the spread, especially given the number of asymptotic carriers.
 
What are we best comparing this to to gauge its severity? I found data from 2017 that refers to deaths by influenza (around 500), pneumonia (around 28k) or in general diseases of the respiratory system (around 74k).

Right now we're at around 2.5k from coronavirus, with the number expected to rise to around 20k or so.

I think if everyone wore them it would be a good thing. It would have to work on a i am protecting you, you are protecting me basis.

While instead of looking at people wearing masks as weirdos, it would be scornful not to have a mask.

My only concern is that mask hygiene is important.

For example ive seen people around the community with masks on and ive seen them taking them of getting into their cars, they grab the front of the mask and pull it off over their face, contaminating their hands and face. You should never touch the front of a mask after use as this is the most likely infected area of a mask, you pull it off by the strings at the back away from your face and wash your hands.

Or ive also seen people pull it off with plastic gloves on, touching and potentially contaminating their faces. That is nothing a bit of education or a 3 minute youtube video wouldn't sort out, as well as how to sanitize and dispose off your mask.

I think the WHO are afraid of that part and the general public to be honest how to actually use this stuff safely, take it off safely and dispose of it safely, there is as much risk of contamination in taking it off and disposing as there is not actually using it, but its not rocket science either, people aren't stupid.

So i think there would need to be a call for the community to wear masks by governments like recommending social distancing, backed by some education on mask hygiene - simple stuff really.

I think it could make a big difference, especially when the community get a bit more closer to reality - going back to work etc, using public transport, shops etc. But id say we wont be seeing it for a while, governments definitely dont want the general public in the medical supply market place.

Here in the US the CDC gives estimated yearly figures on seasonal flu data with 2017/18 a particularly bad year.

CDC estimates that influenza has resulted in between 9 million – 45 million illnesses, between 140,000 – 810,000 hospitalizations and between 12,000 – 61,000 deaths annually since 2010.

Current US estimate given by Dr Fauci for fatalities was iirc 100-240k
