Hard to say without proper diagnostics but I would guess it's a hard drive issue if the OS info can't be located.
Flying blind I would agree that it's probably bad sectors, and that the best bet is to replace the HDD and reinstall the OS.

The bad news is that if that doesn't work, you're looking at replacing things you shouldn't replace, and a new PC is indicated.
 
Fallout (4) is the only game that has truly gripped me. Been playing it on my pc for 4 solid years now, with various mods. Looking forward to London.

Seen the tv series? Superb fun.
It was more a gallows humour type comment that the end of the world seems nigh and we might get to compare an imagined apocalyptic London within days of the real thing. What a time to be running from Deathclaws...
