A skin tag doesn’t suddenly appear and grow to that size. I’m assuming you’ve looked at your arm before and would have noticed it if it’s old...

It looks like a small blister (vesicle). If it’s new and solitary, I’m guessing it’s a reaction to a bite from something. It’ll go away by itself if that’s the case.
