So long as the patient hasn't gone deaf or turned red and has a pulse then you've done well

Questionable...had a high trough...serum creatinine had bumped up I so extended the dosing interval based on the calculated clearance, but obviously hadn't seen the full effect of that SCr elevation because trough was 10 points higher the next day
We're holding now until trough is back in range.

Agreed, but we do a lot of outpatient IV abx to get otherwise stable patients out the door.
