Apparently this is all done by computers using algorithms. There is an ongoing series in Private Eye highlighting these sort of things

Interesting, I used to do some freelance work for local papers around where I live, seem to see more and more things like this, people are usually based to different offices or buildings so advertising see their page space and editorial only see their space but the human eye seems to miss these things unfortunately.
