Have we had a bid accepted for him, to reject us?
The only things I've seen holding up the move are 1) Everton and Juventus haggling over the final number (10 + 25 vs. 10 plus 30 million Euro) and whether it's an option or obligation to buy, and 2) departing players (not just Luiz, but several players on the way out at Juventus) expecting severance payments to agree to a move. Juventus reportedly takes the position that the buying club should pay any bonuses, but if you don't agree to leave, we'll just bench you and you'll miss out on the world cup.

Not the only things though? If the player steadfastly does not want to join Everton, it’s impossible for us or Juve to force him.
 
Luiz has a really clean injury record prior to Juventus, and those "injuries" were common muscular problems in that club's recent history, attributable to excessive strength training without sufficient rest. They have an oafish fitness culture that needs rooting out. They're so backwards in their approach that they've been trying to use "brain training" technology to overcome injuries.

I haven't seen anything about him refusing. In fact, the latest from Juventus-related sites says that Everton is his preferred destination and the only hold up is the option vs. obligation issue.

Not saying this is the case, but Everton could be eyeing someone else as a long term option, say Hayden Hackney, and if that doesn't work out, they're in a position to bite the bullet and agree to an obligation. Or they're holding firm on the option until another club starts looking serious, at which point they'll concede and agree.

It was only a week or so ago that his agent made a big spectacle of flying to the UK to talk to clubs while he stayed in the Brazil and skipped the opening of training. I said then that his agent was trying to get something more from Everton. There still isn't a bid from another English club, and the plan backfired as Luiz was forced to fly back, apologize, and take a 100,000 euro fine.

I think everyone is playing standard negotiation games. Everton are holding out for an option while Moyes publicly complains that deals should have been done by now and they'll need to keep looking, Juventus is holding out for the obligation while leaking that they're willing to hold onto players if terms aren't sufficient, the players push for severance packages knowing the teams need to move them to make other deals, the agents stir up noise in low-level press about new teams coming in. Everyone wants someone to concede something.
