If you're going through a VPN then they can't clock what you're connecting to. There might be stuff like packet data and other words I've heard but don't understand that may suggest you're streaming something but they'd have no way of knowing whether it's IPTV or a more legitimate site such as Youtube, Twitch or XHamster...I think.

Ive got an iptv box, do i need to set up a VPN these days?

No mate not if you get a decent provider.
You can use them on all-sorts of devices using STB app or perfect player.firestick, phone tablet ect.
Hard getting a decent provider do get a trial then take a short payment period say 3 months to start with.
Had a few over the years , most start off good then start to have a few problems .
The one I have now is cheap £35 for a year multiple devices , had a few months of problems , due to illness but in general for what I pay quite happy, loads of sport and VOD.
If you want a VPN something like a zegmma box is easy to set up with one these days.
Haven't used one except a free one on my phone while on holiday, but did it's job
