Heard quite the story today from my other half. She was with an African lady who had just given birth. Holy crap.

My sister in law is a midwife working in Uganda. Over there you have to bring all our own towels, sheets, clothes and everyone is in one big room giving birth.
 

I think that for emergencies (eg brought in by ambulance), then we should treat first, and ask about eligibility later. For non life threatening incidents, a simple NI card or something similar should be produced to prove eligibility.
This seems to work quite well in Australia, when you go to a public hospital, you have to give the triage nurse your medicare card before treatment is given. If it is an emergency, you are treated first and the paperwork sorted out afterwards.
If you are not eligible for free medicare, then expect to recieve a bill.
