The first time I encountered one in the flesh was when I was at uni and took a girl back to my room in halls. The lights were off and I was tipsy and full of beautiful chemicals.
She was sitting on top of me on the bed as she pulled her top off and I unhooked her bra. As I starting exploring her breasts I noticed something was strange about her nipples and thought they must be deformed or something. I didn't want to make her self conscious so I just avoided her tits for the rest of the night. And when I took her home the next time.

It wasn't until when I woke up with her before she got dressed the third time I realised she had pierced nipples.

So I'm probably not the best to give advice on this subject. Although if you want guidance on acting like a gentleman and ploughing a girl with perceived misshapen mammaries then I am very much your man.
